2026-27 School Year
The first step in registering your student for Salem Preschool is to submit the following form for review. (Do not complete the K-12 online registration process until directed to do so by the Salem Preschool.)

Families who already have students enrolled in the Salem School District, should complete a new registration application from the Infinite Campus Parent Portal – this will save time by prepopulating your household infromation we already have on file.
Locate the “More” link, then select “Online Registration”.
If you do not have a parent portal account, please contact the District IT Office at 1-603-893-7069 x5700.
Registration Process (K-12)

All new student registrations start with an online application system. It is recommended you use a computer or laptop. Cell phone use is NOT recommended.
If you have the capability of scanning or photographing documents to electronic form, you will have the option of uploading them during the application process. Otherwise, you will need to complete the application and contact the appropriate registrar to bring in the requested documents.

Step 1: Complete the New Student Online Registration form
The online registration form will require approximately 20 minutes to complete. You should be prepared to enter the following information:
- Household information
- Parent/guardian Information
- Emergency Contacts
- Student Information
- Medication/Immunizations
- Health Physical

The online registration form allows for the direct upload of electronic documents -- either scans or photographs. If you are unable to upload electronic documents, you can still complete the online registration form.
After the registration is submitted and reviewed by one of our registrars, and they will contact you regarding next steps and how to get the forms submitted
Note: Registration Follow Up Meeting (If Necessary)
If you are unable to provide required documents electronically, or you have special circumstances, please complete the application and submit it. Once received and reviewed, you will be contacted by the registrar about next steps.

- Transportation Request
Complete this form if you are requesting bus service from the Salem School District for your student.
- Proof of Residency
A list of current approved documents to verify Salem residency – not older than 30 days.
- Home Language Survey
This form is required for all students.
- Host Residence Form
Form to determine residency if student resides with a host family.
- Physical and Immunization Requirements
A list of immunizations and a physical form to be completed prior to starting school.
- Release of Records
Form to allow transfer of records from your student’s previous school.
- Prescription Medication Administration Request Form
Form to provide information for the administration of prescription medications at school.
